Mandatory Occurrence Reporting

Incidents that have caused or are likely to cause the death or serious injury to a significant number of people, as well as incidents involving structural failure or the spread of fire in the building, must be reported. 

Residents/leaseholders/owners, maintenance personnel, and any other individuals involved with the building must report any known incidents or accidents as soon as they become aware promptly to facilitate proper remediation and reporting.
